The Richard Teller Crane Founder’s Award is presented to Continental Airlines For its outstanding contribution to the development and application of flight operational quality assurance (FOQA), line operations safety audits (LOSA), threat and error management (TEM), and aviation safety action programs (ASAP). The Continental Airlines FOQA program, begun in 1995, provides data that can be used to proactively monitor trends that could affect flight safety. The program significantly enhances line operational safety, as well as training effectiveness, operational procedures, maintenance and engineering, air traffic control procedures and airport-surface procedures. Continental, working with the University of Texas, conducted its first LOSA in 1996 and used the data analysis to develop the “error management” training program. This was followed by the development of TEM, a process used in training, analysis of safety reports, and accident and incident investigations. Recently, Continental completed LOSA 2004. Continental is working with other carriers and the University of Texas to develop an automated ASAP reporting form that will enable a database to be shared within the industry. Representatives of Continental have participated in conferences and workshops with the Air Transport Association, the International Air Transport Association, the International Civil Aviation Organization and Flight Safety Foundation. The activities have promoted “best practices” using safety programs such as those at Continental. Continental has provided, at no cost, LOSA and TEM training for other airlines, U.S. government organizations and a major aircraft manufacturer. Flight Safety Foundation and the award’s sponsor, the Friendship Fund, are pleased to recognize the corporate leadership and contributions of Continental Airlines to aviation safety.
